Optional Supplemental Dental Coverage
2025 Benefits Summary
In 2025, all plan members have preventive and comprehensive dental coverage. Core (HMO) members can purchase additional comprehensive dental benefits through the Optional Supplemental Buy-Up (OSB) for a $22.50 monthly premium. Members must enroll separately.
Once enrolled, present your KelseyCare Advantage Member ID card to your preferred Dental Benefit Providers/UHC Dental provider to start accessing your enhanced dental coverage.

2024 Classic (HMO) Covered Dental Services
While preventive dental benefits are included in your Classic (HMO) plan, the supplemental dental plan is an optional comprehensive plan that covers additional dental services such as fillings, extractions, crowns, root canals, and dentures. The supplemental dental plan is administered by FCL Dental for a monthly plan premium.
Percentage of Covered Dental Expenses Payable
Once the deductible is met, Type II (basic services) and Type III (major services) will be covered at the below coinsurance percentage rates and paid by FCL Dental, up to the Annual Maximum.

Preventive dental services are covered by your KelseyCare Advantage embedded dental plan and, therefore, are not included in the optional supplemental dental plan.
